Form not shown in catagorie and product lists

Hello,

I have a strange problem with a form i made with the form-configurator. I inserted the form in col_4 in the footer. It is showing correctly everywhere, but not in the product and category listings. If i look in the HTML the div is also empty. The form is not limited to store's So it is not a CSS issue or something, it is not rendered.

I have installed the latest version, 3.2.2.3. You can see the settings of the from in the attachment.

Not sure i'm missing something or is this a bug...

I tried some things to resolve the issue but i don't get it to work. I posted te issue on github. https://github.com/s...NET/issues/1906
